Which table this is, and which it is not
The bands are the comparison ETS, which runs TOEFL, publishes between the two tests. Institutions set their own equivalences, and several are stricter than this table: where an institution publishes a TOEFL requirement of its own, that is the one that counts.
PTE Academic and the Duolingo English Test are not included. Both publishers maintain their own concordance with IELTS and both have revised it within recent memory, so any copy here would be the out-of-date one. Read theirs directly.
